- Find out if your provider, hospital or clinic belongs to an HMO, and if so, which
one
- Help you select an HMO over the phone or help you complete an HMO Enrollment Choice
form that you can mail
- Answer your questions about how the HMO works

Forward Health ID Card
The Forward Health ID card is a permanent plastic ID card. Each member of your family
that is eligible for Wisconsin BadgerCare Plus will receive a Forward Health ID
card. You will use the Forward Health ID card every month that you are eligible
for BadgerCare Plus benefits at all types of providers. If your Forward Health card
is lost, stolen or damaged, call Recipient Services toll-free at 1-800-362-3002
for a new card. Take your Forward Health ID card to every doctor, chiropractor
and counseling visit. Health Tradition does not issue a separate medical card for
you to use.
As a member of Health Tradition Health Plan:
- You must visit Health Tradition network doctors, hospitals and clinics for your

hospital near you.
- Once enrolled in Health Tradition, you will receive a new member packet. Call the
Member Advocate at 783-9507 or toll-free at 1-800-545-8499 if you are interested
in attending an enrollment meeting.
- You must choose a primary care clinic when you enroll in Health Tradition. Complete
the Primary Care Clinic Selection Form
and mail to Health Tradition or call the Member Advocate with your selection.
