Prenatal Care Coordination Program
Franciscan Skemp Healthcare and Health Tradition invite you to join Prenatal Care
Coordination (PCC). This program helps you obtain medical and community
resources to have the healthiest possible birth. If you are interested, ask your
provider to refer you to the PCC program or call the PCC office at 608-791-9412

Mayo Clinic Tobacco Quitline
Mayo Clinic Tobacco Quitline is a convenient and confidential phone-based stop smoking
program. It is available to you and your family as part of your healthcare benefits.
Call Mayo Clinic Tobacco Quitline if you would like to talk with a professional
counselor about how ready you are to quit using tobacco and find out how to enroll
in the program.

There are many routine childhood
immunizations that may help to prevent serious diseases.
It is important to keep a record of each time your children are immunized throughout
their childhood. Call your provider to schedule a visit. Health Tradition can design

Calendar

HealthCheck
A HealthCheck is a preventive health-screening program for members from birth to
age 21. The exams are designed to help find possible health problems early when
they can be best treated. HealthCheck also teaches you and your child how to prevent
