Students

Students and parents of students who have Premier, Premier Plus, and Premier One coverage often have questions about what to do for medical care, either as a college student or as a secondary school student (high school). As students become more independent, managing their own health becomes important. Health Tradition has outlined several basic areas students should know about their healthcare coverage. After students reach eighteen years of age, they are an adult for purposes of privacy of medical information and conducting health plan business. This privacy sometimes becomes effective at a younger age. Authorization is required for parents to conduct any business for students over 18 years old or make any inquiries regarding their care or claims.

Out-of-Area Benefit

Covered services for students out-of-area include urgent and emergency care. Premier Plus members have additional services covered with higher member cost-sharing amounts. Premier members require prior authorization for any other service that is not urgent or an emergency, including follow-up services to an emergency or urgent care visit. Please refer to your Certificate of Coverage and your Summary of Benefit information for more details.

Customer Service

For questions on eligibility, medical or pharmacy benefits or services that may require prior authorization, contact Customer Service at 1-877-832-1823 (toll-free). This information is on the back of your ID card for easy reference.

Authorized Representative

To allow another person to inquire about your claims, referrals or care on your behalf, Health Tradition needs an authorization signed by the student granting specific and limited permission. The specific rules are available in your member materials. This form is called "Authorization for Disclosure of Information".

Students can access their claims, view eligibility information, request a membership card and find a physician near you through a secure connection at MMSI Services.com.

Eligibility

You are covered up to age 19 unless in school full-time. Beginning at age 19, each year you will receive a request for an update on your student status. Please complete this form indicating your status or non-status as a student and mail it back to Health Tradition. If a letter is mailed to you inquiring of your student status and is not returned, your claims will not be paid until this information is reported.

Ask Mayo Clinic nurses are available 24 hours a day, 365 days a year for confidential answers to your healthcare questions. The registered nurses will provide you with reliable information or direct you to another resource. The Ask Mayo Clinic phone number is on the back of your ID card as well as in your Member Handbook.
