Health Tradition makes filing claims and billing quick and convenient. Please see the back of your ID card for our filing address as a quick reference. For more details, click the information below.

For questions on the status of a claim, benefits and eligibility, or a particular billing concern, please contact Customer Service toll-free at 1-877-832-1823. They are available from 7 a.m. to 7 p.m., Monday through Friday. A 24-hour voice response unit is also available when you may not need to speak directly with a Customer Service Representative or for calls after hours.

Other Claims Information Online

You can view claims and eligibility information, request a membership ID card, find a physician or pharmacy near you, and determine pharmacy benefits through a secure connection on the MMSI Services Web Site.

The materials on this site should be used to supplement, not replace, your Certificate of Coverage, the Member Handbook and your Summary of Benefit information.

Missing/Needed Forms

You can print a variety of forms the Plan needs to process your claims should you not receive or lose your copy. Click on the Forms tab at MMSIServices.com to see the list of forms available. Some of the most comment forms needed are:

  • Accident (or Injury) Form
  • Pre-Existing Condition Form
  • Dependent Verification Form
  • Request For Medicare Coverage Form
  • SXC Prescription Drug Reimbursement Form
  • Health Reimbursement Account (HRA) Claim Form
