Preventive Care Services
Preventive care, or well care, services and promotion of healthy lifestyles are
central to Health Tradition’s philosophy of care. For this reason, the following
preventive care services are covered at 100 percent when received from network providers:
- Routine preventive exams
- Well-woman gynecological exams
- Well-baby and child care exams
- Adult immunizations
- Child immunizations
- Routine vision exams
- Routine hearing exams

be billed for all other covered services you receive that are not listed as a preventive
service (example: copay, deductible, coinsurance, etc.). Some employers may have

When to Schedule Well Care Visits
Health Tradition has created a helpful chart that tells general timelines for scheduling
well care visits based on your age. The chart includes additional descriptions of
what each exam/test is and how it is useful in reporting your health. Your personal
healthcare provider may modify these guidelines based on your individual needs.

search and learn about the medications you take, how to avoid medication mishaps
and how to get the most out of your medications. You will be able to browse the
most comprehensive listing of books, newsletters and Internet resources available
related to healthcare topics. You will also find health tools that will help you
determine your health status and estimate your risk of disease.

Ask Mayo Clinic nurse line is staffed 24 hours a day, 365 days a year by
experienced registered nurses who draw on the resources of Mayo Clinic to answer
your health questions and help you choose the right level of care for illnesses
and injuries. Ask Mayo Clinic nurse line works to make your life a little
easier by providing:
- A toll-free number you can call any time, day or night, with your health concerns
- Access by phone to friendly, experienced registered nurses who dedicate their knowledge
and time to you
- Reliable health information to help you make informed medical decisions

Mayo Clinic Tobacco Quitline is a telephone-based program designed to help individuals
stop using tobacco. It is an easy-to-use, innovative program designed by physicians,
nurses, researchers and professional counselors at Mayo Clinic to provide needed
support for stopping the use of tobacco.
Mayo Clinic Tobacco Quitline combines the clinical resources of the Mayo Clinic Nicotine Dependence Center and the technical
expertise of Mayo Clinic Tobacco Quitline counselors to offer members the education
and support they need to quit successfully.

Birth by Design
Being informed about your options well in advance of your delivery can help make
your birth experience the best it can be. That's why Franciscan Skemp offers new
moms and dads-to-be a program called Birth by Design.
With Birth by Design, you can make the most of your birth experience by being well
informed about all the choices that are available to you, including:
- Different providers, including obstetricians, family physicians or nurse midwives
- Relaxation, laboring and coping techniques
- Approaches to pain management
- Education and support
- Celebratory meal and gifts
- Newborn care and infant intensive care
- Bringing baby home and ongoing support (such as the Mom and Baby Support Group)
- Arranging for well-baby care with a pediatrician or family care provider
